HEMORRHOID SURGERY - DR. BRUTO RANDONE
Hemorrhoids are varicose veins of the anus. They cause bleeding and pain. They only get worse over time, with constipation, overeating and pregnancy among other factors.
They initially require medical treatment, but at a certain stage they can only be cured with a surgical procedure or a laser treatment. Dr. Bruto Randonee practises both of these techniques.
LASER HEMORRHOID TREATMENT
One of the most important issues in hemorrhoidal surgery is postoperative pain. Laser treatment is a minimally invasive surgical technique that provides a significant improvement in this regard.
Indications
Laser treatment of hemorrhoids (laser hemorrhoidoplasty – LHP) is indicated for symptomatic internal hemorrhoids, particularly grade II and III (it is not indicated for external grade IV hemorrhoids except in certain rare, select cases).
It is generally proposed after failure of medical treatments and instrumental techniques (elastic ligations).
Principle and technique
The treatment is performed on an outpatient basis and involves the introduction of a thin laser fiber (probe) into the hemorrhoidal tissue. The delivered energy causes coagulation of the vessels and retraction of the tissue.
Unlike conventional surgery, there is no excision: the hemorrhoids are reduced from the inside, while respecting the tissue.
Advantages
The main advantage of this technique is very low post-operative pain and faster recovery compared to other techniques.
Disadvantages and limitations
The risk of recurrence after laser treatment is estimated at 15 to 25% in the medium term, potentially reaching 30% in the long term, compared to 5–10% for classic Milligan Morgan surgery, and 15 to 30% for the Longo technique. Effectiveness is limited in the case of significant prolapse (stage IV).
